UWMC Heart Failure Change Teams Present Their Work

During the week of March 13, the Advanced Heart Failure (AHF) Change Teams presented posters at UWMC’s Patient Safety Fair and UW’s Organizational Excellence Showcase. These events provided the Change Teams with the opportunity to highlight their ongoing work and dedication to improving patient care and system processes.

Patient Safety Week was held March 13 – 17, 2017 and posters were displayed in the UWMC main lobby all week to recognize the work being done across the system. The AHF Change Teams presented three posters: Using Patient Safety Principles to Create a High-Functioning, Interprofessional CCU Team on 5SE, Regional Heart Center Outpatient Change Team, and Structured Interprofessional Bedside Rounding. Change Team presenters included, Laura Yale, MHA, Leah Spacciante, RN, MN, and Nancy Murphy, PharmD.

UWs Organizational Excellence (OE) Showcase was held on March 14, 2017 at the Husky Union Building from 1 – 3 pm. The OE annual showcase presents posters of process improvement work happening across the UW, and this year had 55 posters displayed from academic and administrative departments. The AHF Change Teams presented two posters: Regional Heart Center Outpatient Change Team, and Structured Interprofessional Bedside Rounding. Change Team presenters included, Alice Chang, LICSW, CCTSW, Laura Yale, MHA, Nancy Murphy, PharmD, and Susan Pambianco, ARNP.
