Submit a Patient Case Question
How to submit a question about a patient case:
• Complete the online Case Intake Form and by 12pm on the Tuesday before a Wednesday webinar session you plan to attend. Please DO NOT include identifiable information, including patient names, month/day of birth, and medical record numbers.
• Connect to the session 10-15 minutes before the official start time and let us know in advance if you need to present your patient case question at a specific time; we’ll do our best to accommodate that request. When we call on you to discuss your case, make sure to unmute your microphone and turn on your video (if you haven’t done so earlier).
• Provide a brief summary of your case and questions you may have. Peers and specialists will share their thoughts through interdisciplinary discussion. UW consultants will e-mail their suggestions to you at no cost to you or the patient.
