This Online Privacy Statement applies to this website, which is published by the University of Washington (UW) Center for Health Sciences Interprofessional Education, Research and Practice (CHSIE). Collection and Use of Information

The UW Center for Health Sciences Interprofessional Education, Research and Practice may use information collected from your visit(s) to this website to:

  • Manage and improve the user experience and preferences associated with this website;
  • Fulfill your requests for services or materials;
  • Conduct research;
  • Collect and report website use and performance data in aggregate to the UW Board of Health Sciences Deans;
  • Meet legal obligations to grant-funding agencies by collecting and reporting information about users who access certain grant-funded content;
  • Identify anomalies in web traffic in order to help protect personal and UW institutional information; and
  • Exercise the UW’s legal rights, defend against legal claims, or respond to subpoenas, court orders, or other legal processes.

The collection and use of personal information may be applicable to supplemental and more specific notification or consent terms including the European Union’s General Data Protection Regulation (EU GDPR).

European Union General Data Protection Regulation (EU GDPR)

To comply with the EU GDPR, we will provide previously registered users and newly registering users residing in the European Union with our EU GDPR Notification for Collection and Use of Personal Data.  We will request consent to collect and report certain personal data and provide information about how to request full or partial deletion of personal data from their records. 

Personal Data Record and Retention Policies

At a minimum, a full personal data record of a registered user on our website contains a username, email address, current IP address, current user agent (UA), the status of completion for each online learning module registered for, first and last dates and times online learning modules were accessed, quiz scores, and certificates of completion (if applicable). Data records also include any optional personal data that users voluntarily provided upon registration, including first and last name, profession/discipline, affiliated institution(s), how the user heard about our website, or demographic data.  The personal data records of our registered users are stored in a database housed within the UW Center for Health Sciences Interprofessional Education, Research and Practice website and back-up files.

Data records will be maintained for at least their minimum required retention period according to the UW Research and Grant/Contract Records: Research Records and Data schedule available at http://finance.uw.edu/recmgt/gs/research

Collection Methods

The UW Center for Health Sciences Interprofessional Education, Research and Practice may use various technologies such as cookies, applets, scripts, server logs, custom URL parameters, tracking images, information and correspondence, or web analytics to collect information. Depending on your browser you may be able to disable certain collection methods. Some methods of data collection are necessary for the functioning of this website. Opting out of certain data collection methods may result in less than satisfactory user experiences by limiting your ability to use some features of this website. We are working to provide our users with opt-outs when possible.

The UW Center for Health Sciences Interprofessional Education, Research and Practice utilizes a third party website analytics service to collect and aggregate data about how our website is used. We use cookies to track user movement through our website. EU residents have the option to opt out of tracking. 

Disclosure or Sharing of Information

Personal data records may only be accessed by those who have a legitimate UW-related business need to access them. The UW Center for Health Sciences Interprofessional Education, Research and Practice uses third party service providers and suppliers to facilitate this website and the services it provides. Those providers and suppliers may have access to the information the UW collects in order to fulfill their obligations to the UW. The UW is a state agency, and as such may disclose or share information in recognition of applicable state, federal, or international laws. This includes, but is not limited to the Washington Public Records Act, Revised Code of Washington 42.56.

The UW Center for Health Sciences Interprofessional Education, Research and Practice does not report names, usernames, email addresses, identification numbers, IP addresses or other personal identifiers. Data from our tracking service is reported in aggregate to the UW Board of Health Sciences Deans periodically and includes: total page views; page views for individual web pages; number of sessions during a defined time range; navigation paths through our website; types of devices, web browsers and operating systems used to access our website; information requested through user browsers; responses from user browsers; URLs of websites that linked/referred users to our website; and marketing emails that linked/referred users to our website.  

Non-UW Websites

The UW may provide links on this website to other non-UW websites. The UW may also partner with third parties to place UW related ads on non-UW websites. Your use of non-UW websites is subject to the terms and conditions or privacy statements of the providers of those websites.

Data Protection

The UW strives to protect information through measures described in the UW Administrative Policy Statement 2.2 University Privacy Policy, 2.6 Information Security Controls and Operational Practices, and the European Union General Data Protection Regulation (EU GDPR). Due to the rapidly evolving and interconnected nature of information systems and technologies, the protection of information cannot be guaranteed. The UW Center for Health Sciences Interprofessional Education, Research and Practice will notify registered users by email of any possible or actual data breaches as soon as they are detected.

Children or Minors

The UW does not knowingly collect personal information from children or minors as defined by the Children’s Online Privacy Protection Act (COPPA).
